license[s] issued, any convictions for Medicaid or Medicare
fraud, and any disciplinary action or sanctions that may
have been imposed on that physician including details of
the nature of the charges brought against a physician,
the time period involved, this information as gathered
from 70 different Federal and State sources.
The
SECOND REPORT deals exclusively with any past or
present legal suits which may in any way reference Malpractice
as the factor bringing about the suit against the physician.
In each state where the Physician in question was in the
past or is now licensed to practice medicine, a legal
records search for such lawsuits is effected on a county
by county basis within each state. A Report including
the county by county results of the search as well as
a copy of any Malpractice lawsuit is provided.
